According to hotel’s notice, the hotel might require the following documents upon arrival. It may not be possible to stay at the hotel if the following documents will not be presented: 1. Accomplished Complete Health Declaration Form that can be found at the hotel 2. Valid Identification 3. Valid Medical Certificate and COVID 19 RT–PCR Negative Test Result 4. Travel Authority from PNP and Certificate of Acceptance from the Barangay or local government unit.
