A voucher for less than 7 days includes a health block. Sanatorium and resort treatment is prescribed for a voucher for more than 7 days.
In accordance with the Order of the Ministry of Health and Social Development of Russia dated 22.11.2004 No. 256 (as amended on 15.12.2014) "On the Procedure for Medical Selection and Referral of Patients for Spa Treatment" (Registered in the Ministry of Justice of Russia on 14.12.2004 No. 6189), with the Order of the Ministry of Health of Russia dated 05.05.2016 No. 279n (as amended on 25.09.2020) "On Approval of the Procedure for Organizing Spa Treatment" (Registered in the Ministry of Justice of Russia on 21.06.2016 No. 42580), with the Order of the Ministry of Health of Russia dated 15.12.2014 No. 834n (as amended on 18.04.2024) "On approval of unified forms of medical documentation used in medical organizations providing medical care on an outpatient basis, and the procedures for filling them out", the following list of mandatory documents is required for check-in to the sanatorium:

For citizens of the Russian Federation, an identity document is required.
For foreign citizens, all documents permitting stay in the territory of the Russian Federation (passport, visa, migration card) are required;
Sanatorium-resort card in form 072/u-04 (not older than two months).
Sanatorium-resort card can be issued at the Sanatorium for an additional fee.
Voucher (referral or power of attorney from the organization to receive a voucher).
Compulsory medical insurance policy or VHI.
If guests have undergone a medical examination within the last 6 months, then the results of the examinations (ultrasound, FGDS, MRI, CT, laboratory tests, extracts from the outpatient card or medical history).
For pregnant women, an exchange card is required.
Mandatory list of documents for children from 0 to 14 years old:
Children are accepted to the sanatorium of any age, treatment is prescribed for children from 4 years old. Children under 4 years of age are not provided with spa treatment, but for living with parents and being on the territory of the sanatorium, the following documents are required, with the exception of the spa card.
Birth certificate or passport for persons from 14 years of age.
Spa card in the form 076 / u-04.
Compulsory medical insurance policy.
Enterobiasis analysis.
Conclusion of a dermatologist on the absence of contagious skin diseases.
A certificate from a pediatrician or epidemiologist about the absence of contact of the child with infectious patients at the place of residence, in a kindergarten or school.
Consent of legal representatives (one of them) if children are not arriving with their parents.
In the absence of the specified documents, JSC Sanatorium "Actor" has the right to refuse check-in.
The sanatorium recommends that guests additionally take with them:
Clothes and footwear for the weather.
Clothes and footwear for exercise therapy, sports games, visiting procedures and leisure and entertainment activities.
Comfortable shoes for walking along the Terrankur.
Medicines if guests take them regularly.
Swimsuit/swimming trunks, cap and footwear for the pool and balneotherapy.
Personal hygiene items.
What guests need to know about staying at the sanatorium:
All medical procedures are prescribed exclusively by the attending physician of the sanatorium.
It is necessary to arrive on time for the doctor's appointment and for the treatment procedures.
It is necessary to inform the attending physician about any allergies or intolerances to medications.
If guests regularly take any medications, take them with them in sufficient quantities.
To visit water and mud procedures, it is necessary to have bathing accessories, flip-flops, and a cap (for the pool).
A mandatory condition for successful treatment is strict adherence to the doctor's prescriptions, including the treatment program, diet, and physical activity regimen.
To achieve the greatest effectiveness of treatment, it is recommended to abstain from alcohol and smoking.
If guests' health deteriorates or their disease worsens, they must contact their attending physician, in the evening and on weekends, the doctor on duty or the nurse on duty.
